The most common solution (not the only one) is to use a BNC or 'bouncer'. The most commonly used ones are PsyBNC (available for windows and *nix), MUH (*nix only afaik), and a POS called WinBNC. The last one runs on windows only and is very popular but in my opinion its a pile of shit, its unstable and insecure. PsyBNC seems to be the way the world is moving... google for more info.

if U have psybnc on linux there is a efnet server that makes the bnc ip in IPV6 so NoOne can identify that ;)
